I understand that Northside Baptist Church requires a background check or Live Scan for all volunteers over the age of 18.

If I do not already have a current background check or Live Scan on file with the church, one must be completed and approved before I can serve.

I understand and agree that Northside Baptist Church may obtain my personal background information via Live Scan.

Please contact Traci Palmer for information on how to schedule an appointment. Information obtained will be kept confidential.

With my electronic signature on this form, I acknowledge my receipt of a copy of the Live Scan Annual Waiver and agree to allow Northside Baptist Church to continue receiving any updates in regard to my Live Scan reporting as stated within.
